Welcome to Jefferson Manor. Conveniently located in Louisville’s east end, our facility creates a warm, nurturing atmosphere for our patients. We offer services such as Long-Term Care, Short-Term Rehabilitation Services, including Physical, Occupational, and Speech Therapy as well as 24-hour Skilled Nursing Care and Respite Care. We offer both semi-private and private room accommodations and are Medicare certified. After a thorough evaluation, our interdisciplinary care team develops a personalized care plan for you. We then hold regular meetings that keep both you and your family aware of your individualized plan of care.
